Information we collect

Depending on how you interact with Haven, we may collect:

Contact and appointment information: your name, phone number, email address, appointment preferences, and messages you send us.

Information you submit for care: intake forms, medical history, qualifying condition information, identification details, caregiver information, and documents you choose to provide.

Payment information: transaction and billing details. When a payment processor handles card payments, it may collect card details directly under its own privacy terms.

Website information: IP address, browser and device information, pages visited, referring pages, and information collected through cookies or similar technologies.

Please submit medical documents only through a form or communication method Haven designates for that purpose. An ordinary website contact form may not be the appropriate place for detailed medical information.

How we use information

We use information to respond to inquiries; schedule, confirm, and manage appointments; conduct patient intake and provide care; process payments; operate and secure our website; comply with legal and professional obligations; and communicate with you about services you request.

When permitted by law, we may also send practice news or promotional communications. You can opt out of promotional emails using the unsubscribe instructions or by contacting us. Opting out does not stop communications needed for your care, appointments, payments, or other nonpromotional purposes.

Submitting an inquiry or booking an appointment does not guarantee that you will qualify for a physician certification or a Florida medical marijuana registry identification card. A physician makes clinical determinations after an appropriate evaluation.

Florida Medical Marijuana Use Registry

When appropriate for a patient’s care and as required by Florida law, an authorized physician may enter or review information in Florida’s Medical Marijuana Use Registry. The registry is operated under state law, and access to registry information is governed by applicable law. Haven does not control the state’s systems or their privacy practices.

When we share information

We may share information with staff, clinicians, and service providers who need it to perform services for Haven, such as scheduling, website hosting, secure forms, payment processing, and communications. When HIPAA requires it, we use agreements that obligate service providers handling PHI on our behalf to safeguard it.

We may also disclose information for treatment, payment, and health care operations; with your authorization; or when permitted or required by law. These circumstances may include disclosures to the Florida Department of Health or the Medical Marijuana Use Registry, responses to valid legal process, and reports required by law. We do not sell patient medical records or share PHI with third parties for their independent advertising. Uses or disclosures that require your written authorization will occur only with that authorization, subject to applicable law.

If Haven maintains PHI about you, you may also have rights to request access to or correction of your records, request certain restrictions or confidential communications, obtain a copy of our Notice of Privacy Practices, and file a privacy complaint. Those rights and the ways to exercise them are described in the Notice of Privacy Practices.

We will not retaliate against you for raising a privacy concern or filing a complaint.

Security and retention

We use reasonable administrative, technical, and physical safeguards appropriate to the information we maintain. No website or electronic transmission can be guaranteed completely secure. We retain information for the time needed to provide services and meet applicable medical record, legal, accounting, and operational requirements, then dispose of it in accordance with applicable requirements.

Third-party websites

Our website may link to outside websites, including state government, scheduling, or payment websites. Their privacy practices are governed by their own policies. Review those policies before submitting information to them.

Children

Our website is intended for people seeking information about Haven’s services, including parents or legal representatives acting where permitted by law. Children should not submit information through the website without a parent or legal representative. Patient care involving a minor is handled under applicable Florida law.

Changes to this policy

We may update this policy as our services, website practices, or legal obligations change. We will post the revised policy here with a new effective date. Material changes will receive additional notice when required by law.
