We do not collect the state fee for the printed card. The Florida Department of Health OMMU requires an annual fee of $75.00 paid directly to the State through your online registry portal. We assist you during your visit to complete this payment using an online debit or credit card, which expedites your temporary approval email so you can purchase medicine the same day.

Initial Patient Evaluation (Initial in-person examination)
New to Florida MMUR? We guide you through the full application process, including setting up your MMU portal. (State Fee not included). Full refund if not approved. Must be 18+.
Follow-up Re-certification (TELEMEDICINE) Active Certification
(NO Expired Orders). Select if you are a RETURNING patient of HAVENMJD (State Fee not included). Must be 18+.

Follow-up Re-certification (TELEMEDICINE) Expired Certification
(With Expired Orders). Select if you are a RETURNING patient of HAVENMJD. We'll assist with your Florida MMUR application (State Fee not included). Must be 18+.

Request For Exception (RFE) (TELEMEDICINE / In Person)
To request the state approve an increase from the current dosage (for example from 2.5 Oz to up to 5.0 Oz of smoking flower). Note: Some requests may require psychiatric evaluation, with referral and recommendation documentation.
We recommend enrolling and paying online above for a faster visit. Walk-ins are also welcome Monday–Friday 9:00 AM to 2:00 PM at 571/545 Avenue K SE, Winter Haven.
After Your Payment: Complete Your Intake
Once your payment is processed, you will receive an email and text confirmation with a link to the Optimantra Patient Portal. To finalize your enrollment, complete these three steps:
