These Terms describe use of havenmjd.com, appointment scheduling, and nonclinical aspects of services offered by Haven Marijuana Doctor ("Haven"). Patient care is also governed by applicable law, professional duties, our HIPAA Notice of Privacy Practices, and any applicable clinical consent forms. If these Terms conflict with a patient right protected by law, the law governs.
1. Services and clinical judgment
Haven provides medical evaluations related to Florida's medical marijuana program and related follow-up services as offered by the practice. A qualified physician independently determines whether to issue or renew a certification. An appointment, payment, prior certification, or completion of intake forms does not guarantee certification, approval of a requested route or amount, or issuance of a state registry identification card.
Haven is not a medical marijuana treatment center and does not dispense medical marijuana through this website.
2. Website information and emergencies
Website content is general education and does not provide an individualized diagnosis or establish a physician–patient relationship. See our Medical Disclaimer. The website and messaging channels are not emergency services. For a medical emergency, call 911 or go to the nearest emergency department.
3. Accurate information and eligibility
You agree to provide accurate contact, identification, medical history, and appointment information and to update it when needed. A parent, guardian, or other representative must have legal authority to act for a patient. Haven may request documentation needed to verify identity, authority, residency, or eligibility under Florida law.
4. Appointments and telemedicine
Appointments are subject to clinician availability and applicable law. An initial Florida medical marijuana certification requires an in-person examination. A renewal visit may be conducted through telehealth only when the statutory conditions and clinical requirements are met. Haven may request records, additional evaluation, or an in-person visit before making a clinical decision.
5. Fees, cancellations, and refunds
Current service prices will be shown at booking or otherwise communicated before payment. State identification card fees, dispensary purchases, and third-party costs are separate unless Haven expressly states otherwise.
Haven's separately published Refund and Cancellation Policy governs cancellations, missed appointments, and refunds. If a payment or fee term shown during checkout differs from these general Terms, the term specifically disclosed for that transaction will apply to the extent permitted by law.
6. Communications and privacy
Our Privacy Policy describes website information practices; our HIPAA Notice of Privacy Practices describes how we use and disclose protected health information. Text messaging is governed by our SMS Privacy Policy and Messaging Terms. Do not use ordinary email, texts, or website contact forms for emergencies or detailed medical submissions unless Haven directs you to an approved channel.
7. Third-party services
Links to the Florida Department of Health, the Medical Marijuana Use Registry, payment processors, or other external sites are provided for convenience. Those organizations control their own services, fees, and policies. Haven cannot guarantee their processing times or decisions.
8. Results and availability
Medical outcomes vary. Haven does not guarantee symptom improvement or a particular certification decision. We may change website content, scheduling availability, and future service offerings; changes do not alter patient rights or obligations already established by law or a completed transaction.
9. Appropriate website use
You agree not to submit false information, attempt unauthorized access to the website or patient systems, interfere with their operation, or misuse another person's identity or records. Haven may restrict website access when reasonably necessary to protect security or comply with law; clinical continuity and access to records will be handled under applicable professional obligations.
10. Ending care
You may choose to stop receiving services. Haven may end a treatment relationship when permitted by professional and legal requirements, including when safe or appropriate care cannot continue. Any decision to end care will be handled with appropriate notice and continuity arrangements required by applicable law. These Terms do not set a fixed period of emergency care.
11. Governing law and contact
Florida law governs these Terms to the extent applicable. Any dispute provision is subject to mandatory patient protections and applicable jurisdictional rules. Nothing in these Terms waives a claim for professional negligence or limits a remedy that cannot lawfully be limited.
We may revise these Terms for future website use or transactions by posting an updated version with a new effective date.
