How to Release Your Current Physician
Welcome as a transfer patient! To complete your transfer to Haven Marijuana Doctor, the State of Florida requires that you first release your current certifying physician in the Medical Marijuana Use Registry (MMUR). Only you — the patient (or your legal representative) — can authorize this change. Use the steps below.
Steps to Release Your Physician

Go to the official Florida Medical Marijuana Use Registry and log in with your patient account: mmuregistry.flhealth.gov

Once logged in, open your account and locate your current certifying physician under your profile or orders.

Select the option to release (remove) your current physician from your record. Confirm the change when prompted.

After the release is confirmed, your file is free to be claimed by a new certifying physician.

Visit Haven Marijuana Doctor (in person at 571/545 Avenue K SE, Winter Haven, FL) for your scheduled transfer appointment. Our licensed physician will claim your record and enter your new certification directly into the MMUR.
If you are unable to release your current physician
If you cannot release your current physician online — for example, you cannot access your registry account, you forgot your password, or the option is not showing — you have two options:
Option 1 — Call the Office of Medical Marijuana Use (OMMU)
Call the Florida Office of Medical Marijuana Use (OMMU) directly. Helpline: 800-808-9580 (weekdays, 7:00 AM – 6:00 PM EST). Main line: 850-245-4657. A state representative can release your current physician or help you regain access to your registry account.
Option 2 — We Help You in Our Office
We can help you with the process in our office. Bring a valid photo ID to 571/545 Avenue K SE, Winter Haven, FL during walk-in hours (Monday–Friday, 9:00 AM – 2:00 PM) or your scheduled appointment, and our staff will guide you through releasing your current physician in the registry.
Your existing Florida registry ID and medical marijuana card remain valid during the transfer — you do not pay any additional state fee to transfer, and your current orders stay active until your new physician updates them.
